What we collect
- What you send us: the details you type into a form, such as your name, email, borough, language and message. We do not need medical details, so please don’t include them.
- Proof of your consent: the consent wording you agreed to, the date and time, the page, and your IP address.
- How the site is used: pages visited and buttons tapped, measured with Google Analytics without your name, email or messages.
Why we use it
- To answer your question by email.
- To send your message to a company, only when you choose that company and agree to share with it.
- To understand which pages help families, so we can improve them.
Who sees it
The cdpap.nyc team; the one company you choose to contact, if any; and the services that keep the site running, such as our web host and email provider, only as needed to provide them.
What we never do
- Sell or rent your information.
- Share it with a company you did not choose.
- Share health or Medicaid information with advertising platforms, or use it to target ads.
How we contact you
We reply by email to the address you give us. We do not call or text you.
How long we keep it
Form messages and consent records are deleted after 12 months, unless we are still helping you.

The site uses encrypted connections (HTTPS), and only the cdpap.nyc team can see form messages.
