How to apply for CDPAP in New York City

There is no single CDPAP application. You get Medicaid, get assessed, get hours approved, then register with PPL. Here is each step in order, with who to contact.

Step 1: Get Medicaid

CDPAP is paid by Medicaid, so the person who needs care must have New York Medicaid that covers home care. If they already have it, go to step 2.

Step 2: Request the assessment

  • Regular Medicaid, no health plan: call the New York Independent Assessor Program at 1-855-222-8350 (TTY 1-888-329-1541). You can also contact HRA, which will call the assessor program with you on the line.
  • Already in a Medicaid health plan: call your plan first. It will refer you for the assessment.

The assessment has two parts, in person or by video: a Community Health Assessment of about 2 to 3 hours, and a clinical appointment of up to 1 hour, where a practitioner completes the order for services. Have a list of medications, doctors and the daily activities the person needs help with.

Step 3: Get the hours approved

After the assessment, hours are approved by whoever manages the person’s Medicaid:

  • Managed long term care (MLTC) plan: adults 21 and older with both Medicare and Medicaid who need home care for more than 120 days generally must join one. New York Medicaid Choice, at 1-888-401-6582, helps you choose. MLTC plans cover consumer directed care.
  • Medicaid health plan: the plan approves the hours.
  • Regular Medicaid with no plan: HRA approves the hours.

Ask for consumer directed personal assistance by name, so the approval is for CDPAP and not agency home care.

Step 4: Choose a facilitator and a caregiver

Pick the caregiver you want to hire. Check who is allowed. Then choose a facilitator to help with registration, or register directly with PPL. Facilitators are local organizations that work with PPL, and you can choose any one. To make the choice official, use the Department of Health’s facilitator selection form.

Step 5: Register with PPL and start

  1. Register through PPL@Home online, through your facilitator, by email at [email protected], by phone at 1-833-247-5346, or at a PPL office by appointment.
  2. Both the person getting care and the caregiver register. Have the Medicaid ID (CIN) ready. It is usually eight characters, like AB12345X, and is on the Medicaid card.
  3. The caregiver completes the offer letter, tax forms, Form I-9, a payment method form and the health assessment.
  4. PPL tells you the start date. The caregiver then records hours each week and is paid every Thursday.

How long it takes

There is no official timeline. The Medicaid application, scheduling the assessment, the plan’s decision and the paperwork all add time. Keep copies of every notice and respond quickly to keep each step moving.

Common questions

Can I apply for CDPAP online?

Parts of it. You can apply for Medicaid online through ACCESS HRA or NY State of Health, and register with PPL online through PPL@Home. The assessment is requested by phone.

Who do I call first?

If the person has Medicaid and no health plan, call the New York Independent Assessor Program at 1-855-222-8350. If they are in a Medicaid plan, call the plan. If they do not have Medicaid, apply for it first.

Can I switch from agency home care to CDPAP?

Yes. The choice between CDPAP and agency personal care belongs to the person getting care. Tell the plan or HRA you want consumer directed care, then register with PPL.

What if we are denied?

You can appeal. With a Medicaid plan, ask the plan for an appeal first, then a state fair hearing if needed. More about denials.
