  • Bronx Independent Living Services (BILS): the Bronx’s independent living center, run by and for people with disabilities, with services in English, Spanish and ASL. Phone 718-515-2800. bils.org
  • NYC Aging, Aging Connect: caregiver support, respite and referrals for older New Yorkers and their families. Call 212-244-6469 (212-AGING-NYC).
  • ICAN: New York’s free, state-funded ombudsman for people with Medicaid long-term care, including managed long term care plans. Call 1-844-614-8800.
  • Medicaid applications: people 65 and older or with a disability apply through ACCESS HRA or at 347-396-4705; most others apply through NY State of Health at 1-855-355-5777.

Starting CDPAP in the Bronx

  1. Make sure the person needing care has Medicaid that covers home care.
  2. Request the assessment from the New York Independent Assessor Program at 1-855-222-8350, or from the Medicaid plan.
  3. Get the hours approved by the plan or HRA, and ask for CDPAP by name.
  4. Choose a facilitator and register with PPL.

Do I have to use a facilitator in the Bronx?

No. You can choose any PPL facilitator that serves the Bronx, or register directly with PPL. A local office only matters if you want to meet in person.
