Starting CDPAP in Brooklyn
- Make sure the person needing care has Medicaid that covers home care.
- Request the assessment from the New York Independent Assessor Program at 1-855-222-8350, or from the Medicaid plan.
- Get the hours approved by the plan or HRA, and ask for CDPAP by name.
- Choose a facilitator and register with PPL.
